Most Ghanaians have never had a cholesterol test. Yet high cholesterol is one of the biggest hidden health risks in Ghana today — silently building up inside the arteries of people who feel perfectly fine, until a heart attack or stroke strikes without warning. The good news is that high cholesterol can be detected with a simple blood test, managed with changes to what you eat, and treated if needed. This guide explains everything in plain language — starting from the very beginning.

## What Is Cholesterol? A Plain Language Explanation

Cholesterol is a fatty substance that your body needs to function. It is found in your blood and in every cell of your body. Your liver makes most of the cholesterol your body needs — you do not have to eat any at all for your body to have enough. Cholesterol helps build cell walls, make hormones like oestrogen and testosterone, and digest food.

The problem is not cholesterol itself. The problem is when there is **too much of a certain type** of cholesterol in your blood. When that happens, it starts to stick to the walls of your arteries \(the blood vessels that carry blood from your heart to the rest of your body\), forming a build-up called **plaque**. Over time, this plaque narrows the arteries, restricts blood flow, and can eventually block a blood vessel completely — causing a heart attack or stroke.

Think of it like a water pipe. A clean pipe has water flowing freely. A pipe with build-up on the inside walls has a smaller opening, lower pressure, and eventually may block completely. Your arteries work the same way.

## Good vs Bad Cholesterol: What HDL and LDL Actually Mean

When your doctor talks about cholesterol, they usually refer to two types: **LDL and HDL**. Understanding the difference is straightforward:

LDL

Bad Cholesterol

Low-Density Lipoprotein

  * ✕ Sticks to artery walls and builds up as plaque
  * ✕ Narrows blood vessels over time
  * ✕ Raises risk of heart attack and stroke
  * ✕ Raised by fried foods, red meat, palm oil

Keep this LOW

HDL

Good Cholesterol

High-Density Lipoprotein

  * ✓ Collects excess cholesterol and takes it to the liver
  * ✓ Cleans the arteries from the inside
  * ✓ Protects against heart disease
  * ✓ Raised by exercise, fish, avocado, nuts

Keep this HIGH

A simple way to remember it: **LDL = Lousy cholesterol** \(you want it low\). **HDL = Healthy cholesterol** \(you want it high\). Your cholesterol test will show both numbers, plus a total. Your doctor uses these numbers together to assess your heart risk.

Did You Know?

High cholesterol produces no symptoms at all. You cannot feel it, see it, or taste it. The only way to know your cholesterol level is to have a blood test. This is why so many Ghanaians are walking around with dangerously high cholesterol and have no idea.

## Ghanaian Foods That Raise Your Cholesterol

Many of the most popular foods in Ghana are high in **saturated fat** — the type of fat that raises LDL \(bad\) cholesterol in the blood. This does not mean you must stop eating them entirely, but it does mean knowing which foods to reduce, especially if your cholesterol is already high.

  * **Palm oil \(red oil\).** Used in almost every Ghanaian dish — light soup, stew, kontomire, palm nut soup. Palm oil is very high in saturated fat. Cooking with large amounts every day raises LDL cholesterol significantly. Use smaller quantities or mix with healthier oils like sunflower or olive oil.
  * **Red meat and organ meat.** Beef, goat, and especially offal \(liver, tripe, intestines\) are high in saturated fat and dietary cholesterol. Eating these in large portions regularly raises LDL levels.
  * **Fried foods.** Kelewele, fried yam, fried chicken, fried fish — frying food in hot oil \(especially reused or palm oil\) increases the saturated and trans fat content significantly.
  * **Processed and packaged foods.** Canned meats, sausages, instant noodles, and biscuits often contain hidden saturated and trans fats that raise LDL cholesterol.
  * **Full-fat dairy.** Whole milk, butter, and cheese contain saturated fat. These are becoming more common in Ghanaian diets, especially in urban areas.
  * **Coconut milk.** Popular in some Ghanaian soups and rice dishes. Coconut is very high in saturated fat — use in moderation.

## Ghanaian Foods That Help Lower Your Cholesterol

The good news: many traditional Ghanaian foods are excellent for your heart and can help lower LDL cholesterol naturally when eaten regularly.

  * **Beans and legumes \(beans stew, cowpea, groundnuts\).** Beans are rich in soluble fibre, which binds to cholesterol in the digestive system and removes it from the body before it enters the bloodstream. Eating beans regularly is one of the most effective dietary ways to lower LDL.
  * **Oats \(oatmeal porridge\).** Oats contain a special type of fibre called beta-glucan that directly lowers LDL cholesterol. A bowl of plain oat porridge for breakfast is one of the easiest changes you can make.
  * **Avocado \(pear\).** Avocado is high in monounsaturated fat — the heart-healthy type that raises HDL \(good\) cholesterol and lowers LDL. Widely available in Ghana and very affordable.
  * **Fish.** Tilapia, mackerel \(Kpanla\), herring, sardines, and tuna contain omega-3 fatty acids that lower triglycerides and reduce heart disease risk. Grilled or boiled fish is far better than fried fish for your cholesterol.
  * **Kontomire \(cocoyam leaves\) and other dark leafy vegetables.** Rich in fibre, antioxidants, and plant sterols that help manage cholesterol levels.
  * **Garden eggs \(eggplant\).** A traditional Ghanaian food that contains soluble fibre and antioxidants beneficial for cholesterol control.
  * **Dawadawa \(locust beans\).** A fermented seasoning traditionally used in Ghanaian cooking that has been shown in studies to have cholesterol-lowering properties.

## How High Cholesterol Leads to Heart Attack and Stroke

High cholesterol does its damage slowly and silently over years. Here is exactly how it happens:

  1. **LDL cholesterol builds up in the artery walls.** When there is too much LDL in the blood, it seeps into the walls of the arteries and gets trapped there.
  2. **Plaque forms.** The trapped LDL combines with other substances to form a hard deposit called plaque. This plaque makes the artery wall thicker and the channel inside narrower.
  3. **Blood flow is restricted.** As plaque grows over years, less blood can pass through. The heart has to work harder to push blood through the narrowed vessels, which also raises blood pressure.
  4. **The plaque can rupture.** Sometimes a piece of plaque breaks off. This triggers the body to form a clot at the site of the rupture, which can completely block the artery.
  5. **Heart attack or stroke.** If the blocked artery supplies the heart, the result is a heart attack. If it supplies the brain, the result is a stroke. Both can happen with no warning — even in people who felt completely healthy.

This connection between cholesterol, high blood pressure, diabetes, and stroke is why doctors always check cholesterol levels alongside blood pressure and blood sugar. These three conditions often occur together and multiply each other's damage.

## How to Get a Cholesterol Test in Ghana

A cholesterol test — called a **lipid profile** or **lipid panel** — is a simple blood test. You fast \(eat nothing\) for 9 to 12 hours before the test, then a small sample of blood is taken from your arm. Results are usually ready within a day. The test shows your:

  * **Total cholesterol** — the overall amount in your blood
  * **LDL \(bad\) cholesterol** — the number to keep low
  * **HDL \(good\) cholesterol** — the number to keep high
  * **Triglycerides** — another type of blood fat linked to heart disease

You can get a cholesterol test at:

  * **Any hospital or polyclinic** with a laboratory — including NHIS-accredited public hospitals where the cost may be covered
  * **Private diagnostic laboratories** across Accra, Kumasi, Takoradi, and other major towns. Typical cost: **GHS 80–200** for a full lipid profile
  * **Private clinics and health centres** — many now include cholesterol testing as part of routine health checks

**Who should get tested?** Every adult over 35 should have at least one cholesterol test. If you have high blood pressure, diabetes, are overweight, or eat a lot of fried or fatty foods, get tested now regardless of age. If your result is normal, retest every 3 to 5 years. If it is high, your doctor will advise on how often to retest.

## Frequently Asked Questions About Cholesterol in Ghana

What is a normal cholesterol level in Ghana? +

Doctors generally use international guidelines. A healthy total cholesterol level is below 5.0 mmol/L \(200 mg/dL\). LDL \(bad\) cholesterol should ideally be below 3.0 mmol/L. HDL \(good\) cholesterol should be above 1.0 mmol/L for men and 1.2 mmol/L for women. Your doctor will interpret your result based on your overall health, blood pressure, age, and whether you smoke or have diabetes.

What are the symptoms of high cholesterol in Ghana? +

High cholesterol has no symptoms. You cannot feel it. There is no pain, no tiredness, no visible sign. The only way to know your cholesterol level is through a blood test. This is why it is called a silent condition — by the time symptoms appear \(such as a heart attack or stroke\), the damage has already been done over many years.

Is palm oil bad for cholesterol? +

Palm oil contains a high amount of saturated fat, which raises LDL \(bad\) cholesterol when consumed in large quantities regularly. This does not mean you must stop using it entirely — palm oil is part of Ghanaian food culture — but using smaller amounts, not reusing old oil for frying, and balancing your diet with cholesterol-lowering foods \(beans, oats, fish\) will help manage the impact.

Can you lower cholesterol without medicine in Ghana? +

Yes, if your cholesterol is mildly to moderately elevated, dietary changes and exercise can lower it significantly without medication. Eating more beans, oats, and grilled fish, reducing fried foods and large amounts of palm oil, losing weight if you are overweight, and exercising at least 30 minutes most days are all evidence-based ways to improve cholesterol levels. If lifestyle changes are not enough, or if your cholesterol is very high, your doctor may prescribe medication.

Does high cholesterol cause heart attack and stroke? +

High LDL cholesterol is one of the leading causes of heart attack and stroke. It does this by slowly building up plaque inside the arteries over years, narrowing them until blood flow is restricted or blocked entirely. Combined with high blood pressure or diabetes — both of which are common in Ghana — high cholesterol multiplies the risk dramatically. Getting tested and managing your levels early is the most effective prevention.

How much does a cholesterol test cost in Ghana? +

A full lipid profile \(cholesterol\) blood test at a private laboratory in Ghana typically costs between GHS 80 and GHS 200. At NHIS-accredited public hospitals, the test may be partly or fully covered by the National Health Insurance Scheme. You need to fast for 9 to 12 hours before the test for the most accurate result.
